If you or your family member would like to be evaluated for admission to Acuity Hospital of Houston, please call Rudy Jaesnch in our Admissions Office at 281.921.5303. If you are currently a patient in a hospital, please talk with your physician or Case Manager.
Once your physician has referred you to Acuity Hospital of Houston, a clinical liaison will visit you in the hospital. She or he can explain the transfer process and answer any questions you may have.
It is our goal to make the admission process as streamline as possible. You or your family will need to provide your insurance card and complete admission paperwork. The admission paperwork is designed to inform you of your rights as a patient and provide our staff with your contact information. Acuity Hospital of Houston encourages patient and family involvement in the patient's Plan of Care. Our clinical team holds weekly Patient Care Conferences to assess a patient's progress toward their goals. Acuity recognizes that each patient is unique; therefore each patient's Plan of Care is customized to meet the needs of the patient.
